Achieving Patient-Centered Coordinated Care: Bundling and the Future of Home and Community Based Care
Join the Alliance for a discussion on payment and health care delivery reforms to achieve more coordinated, efficient and cost-effective care for our nation’s seniors, and how these reforms will relate to the future of home health care. As more baby boomers enter retirement, reforms will be needed to foster patient-centric care models that reward the value of services over volume. Congressman David McKinley and Douglas Holtz-Eakin will discuss the reforms needed to improve care and shore up the future of the Medicare program, including the potential use of bundled payments in post-acute care to accomplish these goals. Contextualizing bundling in the context of the future home health care and health care delivery writ-large, Institute of Medicine Program Director Tracy Lustig will discuss highlights from the IOM’s workshop on “the Future of Home Health Care” and how home and community based care should be considered in the future to improve care for older Americans and those with disabilities.
